Lipoedema surgery at a glance

What lipoedema is, when surgery after weight loss makes sense, how the procedure and healing work – and under what conditions German health insurance pays.

What is lipoedema – and what can surgery do?

Lipoedema is a chronic, painful fat distribution disorder: fatty tissue increases abnormally, usually symmetrically on the legs and hips, often also on the arms and buttocks. The fat deposits are resistant to diets and exercise, often remain even after significant weight loss, and frequently lead to swelling, pain and considerable physical and emotional strain. Typical features are a symmetrical increase of fatty tissue on the arms and/or legs and pressure and touch pain in the tissue of the affected regions; hands and feet are not affected and remain slim.

Lipoedema surgery is not conventional liposuction but a specialised procedure: it removes the diseased fatty tissue in a targeted way and spares the healthy tissue. It can relieve pain; it is not intended for further weight loss. Your lipoedema surgery is part of Weight Doctors® RESHAPE: with an individual treatment plan and support from the first consultation to aftercare.

Who is lipoedema surgery suitable for?

A bariatric operation or conservative weight loss melts away the healthy fat – the lipoedema fat remains. Especially after a bariatric procedure it therefore often only becomes properly visible, because the other fat deposits have gone. The pain in the arms and legs also remains. This is where lipoedema surgery can come in.

With a BMI of 35 or above, obesity has to be treated first before lipoedema surgery. At Weight Doctors® you receive both with continuous support: first you reduce your weight, for example with Weight Doctors BARIATRICS®, the Weight Doctors SLEEVE® or GLP-1 pharmacotherapy, followed by lipoedema surgery as part of Weight Doctors® RESHAPE.

Whether lipoedema surgery is suitable for you is clarified by the doctor in the initial consultation after a physical examination. It is recommended if:

Doctor explaining a treatment to visitors at the Weight Doctors® open day
  • after a bariatric operation or conservative weight loss, painful, diseased pockets of fat continue to exist that cannot be reduced despite weight loss
  • your weight is stable and your general health is good
  • you have realistic expectations: the aim is fewer symptoms and a more balanced contour, not further weight loss
  1. BMI from 35treat obesity first, then lipoedema surgery
  2. BMI from 40no liposuction through German statutory health insurance

How lipoedema surgery works in practice

Depending on the extent, lipoedema surgery takes 1.5–3 hours. This is how it proceeds:

Surgeon and theatre team during liposuction
  1. Diagnosis and planningIt begins with a detailed conversation with a doctor experienced in the diagnosis and treatment of lipoedema. After your medical history and a physical examination, the severity and affected regions are determined. The treatment plan depends on the stage, the body regions and your goals; the techniques are also discussed: tumescent technique, water-jet-assisted (WAL) or vibration-assisted liposuction (VAL).
  2. PreparationBefore the procedure there is a blood test and advice on diet, medication, alcohol and nicotine. In some cases manual lymphatic drainage is recommended beforehand to reduce the fluid in the tissue.
  3. ProcedureOn the day of surgery you arrive having fasted; after a final discussion the areas are marked. Depending on the extent and your wishes, the operation is carried out under general anaesthesia or under local anaesthesia with sedation. First a larger quantity of tumescent solution containing local anaesthetic and adrenaline is introduced: it numbs, constricts the blood vessels, reduces bleeding and loosens the tissue. Then the diseased fat is suctioned out through small skin incisions with a thin cannula – as gently as possible for the surrounding tissue, the lymphatic vessels and the skin. How much is suctioned out depends on the severity and the region treated.
  4. After surgeryThe areas are dressed with compression bandages or garments, and you are monitored for a few hours. After general anaesthesia you usually stay in the clinic for one night; after local anaesthesia you can often go home the same day.
  5. AftercareYou wear compression garments for at least six weeks. Manual lymphatic drainage can support the drainage of lymph; follow-up appointments accompany healing.

After the operation: compression, lymphatic drainage and check-ups

For the first days and weeks you receive precise instructions – on wound care, on painkillers and antibiotics and on wearing compression. After the procedure you receive access to Weight Doctors® ShapeWear, compression garments fitted to your body; during the therapy a Weight Doctors® 360 coach is also at your side. Follow-up appointments ensure that possible complications are detected early. This is how healing typically proceeds:

  1. Day of surgeryMonitoringAfter local anaesthesia often home the same day.
  2. Day 1After general anaesthesiaAs a rule one night of observation in the clinic.
  3. first weeksLymphatic drainageGentle massages can promote lymph drainage and reduce swelling.
  4. min. 6 weeksCompressionThe compression garments support the skin and help prevent bruising.
  5. 3–6 monthsResultSwelling has subsided, the tissue has adapted.

How effective is lipoedema surgery after a bariatric procedure?

While a bariatric operation promotes weight loss, the lipoedema fat often remains and continues to cause symptoms. Studies and the experience of many patients show that lipoedema surgery can reduce pain and improve the body contour – and with it often also quality of life and self-esteem. Results vary from person to person.

So that the result lasts, pay attention to a balanced diet and regular exercise after the operation too; the progress is documented by the Weight Doctors® 3D Bodyscan. The decision for the procedure should be carefully considered and made together with a specialist. Look for experience with lipoedema operations, especially after major weight loss, verified qualifications and an open conversation about your expectations.

Costs and German health insurance: requirements and application

Where the medical indication meets the guidelines, cost coverage by German health insurance is possible. In connection with cost coverage, lipoedema surgery starts at €950. As a self-paying patient, the price depends on the extent and the number of regions; you receive an exact cost estimate after the initial consultation.

German statutory health insurance covers lipoedema surgery under defined conditions – usually for lipoedema at an advanced stage (stage 3). The diagnosis must be made by a specialist in internal medicine and angiology, in physical and rehabilitative medicine or in dermatology – or by a specialist with additional training in phlebology.

The insurer also requires medically prescribed conservative therapy, such as lymphatic drainage, compression or physiotherapy, which has run continuously for at least 6 months without helping sufficiently. During these 6 months you must not have gained weight, documented monthly by your GP; psychological factors are also recorded. Severe pain, restricted movement or other health problems can likewise be recognised as medical necessity. From a BMI of 40, liposuction through German statutory health insurance is not possible.

For cost coverage by German health insurance, an application is needed with the diagnosis, evidence of the therapy and a medical certificate of medical necessity. You do not have to submit it yourself: we check the requirements and policy terms with you, compile the cost estimate and evidence, and submit everything. With private health insurance, approval depends on your policy and possible exclusions; some insurers have their own lipoedema programmes. You can read all the details in the article Lipoedema surgery: how cost coverage works.

What German statutory health insurance covers – and what it does not:

  • This is paid by the German health insurerLiposuction on the arms, legs and, depending on the findings, the buttocks, plus anaesthesia, inpatient monitoring and the usual aftercare.
  • This is not paid by the insurerLiposuction of the abdomen, back or chest, and skin tightening after liposuction.

Frequently asked questions

Questions about Weight Doctors® Lipoedema Surgery

Your question isn’t listed? Your specialist will take time for it during the consultation.

What is lipoedema?

A chronic fat distribution disorder in which fatty tissue increases abnormally, usually symmetrically on the legs and hips, often also on the arms and buttocks. Typical features are swelling and pain on pressure or touch; hands and feet remain slim.

Does losing weight help against lipoedema?

Hardly against the diseased lipoedema fat – it does not respond to diet and exercise. Losing weight is still important if there is also excess weight: from a BMI of 35, obesity is treated first, then the lipoedema.

Is lipoedema surgery ordinary liposuction?

No. Lipoedema surgery is a specialised procedure that removes diseased fatty tissue in a targeted way while sparing lymphatic vessels and healthy tissue as far as possible. Aesthetic liposuction, by contrast, shapes stubborn but healthy fat deposits.

How long does lipoedema surgery take?

Depending on the extent, 1.5–3 hours, under general anaesthesia or under local anaesthesia with sedation.

Is lipoedema surgery carried out as an outpatient or inpatient?

Both are possible. After general anaesthesia you usually stay one night for observation; after local anaesthesia you can often go home the same day.

How long do I have to wear compression after the operation?

At least six weeks, depending on your doctor’s instructions. The compression garments reduce swelling, help the skin adapt and help prevent bruising and uneven results.

Do I need lymphatic drainage after the operation?

It is often recommended. Gentle manual lymphatic drainage supports the drainage of lymph fluid and can reduce swelling further. In some cases it is also used before the operation.

How long does healing take, and when can the result be seen?

Swelling and bruising usually subside within a few weeks. The final result becomes apparent after about three to six months, once the swelling has subsided completely and the tissue has adapted. Results vary from person to person. Until then, avoid physical exertion, wear the compression garments and follow your doctor’s instructions exactly.

What risks and side effects can lipoedema surgery have?

As with any surgical procedure, risks and side effects are possible: swelling, bruising, infections and irregular skin contours, in rare cases thrombosis or wound-healing problems. Choosing an experienced specialist and careful aftercare can reduce the risk; it is important to follow all medical instructions exactly.

Who may perform lipoedema surgery?

Specialists in plastic, reconstructive and aesthetic surgery, surgically active specialists in other surgical fields, and dermatologists. At Weight Doctors® RESHAPE, specialists in plastic and aesthetic surgery operate – all under Our specialists.

What does lipoedema surgery cost?

With cost coverage by health insurance from €950. As a self-paying patient, the price depends on the extent and the number of regions; you receive an exact cost estimate after the initial consultation, for which a fee of €30 applies under the German medical fee schedule (GOÄ). All prices: Costs & prices.

Does German statutory health insurance cover the costs?

Yes, under certain conditions: diagnosis by an approved specialty, at least 6 months of documented conservative therapy without sufficient improvement, a stable weight and a BMI below 40. Your German health insurer decides on a case-by-case basis.

Does private health insurance pay?

That depends on your policy. If lipoedema is included in the scope of benefits, the insurer needs the diagnosis, evidence of therapy, evidence of a stable weight, a medical certificate and a cost estimate from the clinic.

Do I have to submit the application to the German health insurer myself?

No. We check the requirements with you, compile all evidence and the cost estimate, and submit the application.

Where does lipoedema surgery take place?

With cost coverage by German health insurance in the Weight Doctors® Nationalklinik in Wertheim. As a self-paying patient at all locations offering Weight Doctors® RESHAPE, for example in Dortmund, Düsseldorf, Hamburg, Nuremberg or Eindhoven.
